Volunteer Application Forms
Service opportunities are available year round at California Hospital Medical Center. Volunteers are asked to commit to a weekly, bi-weekly, or monthly schedule. Adult community members, college students and vocational education students are welcome to apply throughout the year.
Interested candidates must attend a Volunteer Introductory Meeting on Saturday, May 6, 2017, at 10a.m. in the hospital's Command Center. We will collect your application at that time, and answer any questions you may have.
Adult applicants must provide a social security number in order to obtain a mandatory background screening. All candidates must complete a health screening. Screenings are provided free of charge by California Hospital Medical Center.

High School Students
High School students must commit to 12 months of participation. Applications for high school students (ages 14-17) are distributed at the conclusion of a mandatory “Introductory Meeting”. Minor high school students are required to attend with a parent or guardian (brothers and sisters are not acceptable substitutes). Following attendance at the “Introductory Meeting”, prospective volunteers are invited to interview with the Volunteer Services staff. General Volunteer Orientation takes place once a year, in the spring.

Monday, February 27, 2017 from 6pm -7pm in Keck Hall
Tuesday, March 7, 2017 from 6pm – 7pm in Keck Hall
Thursday, March 9, 2017 from 6pm – 7pm in Keck Hall
General Orientation- By invitation only. Mandatory attendance, no exceptions.
June 10, 2017 at 10:00am – 2:00pm
All meetings are held at California Hospital Medical Center, Keck Hall, (located in Leavey Hall, 320 W. 15th St., Los Angeles, CA 90015).

Candidates who submit a completed application packet will be invited to a personal interview. Interviews are scheduled in the order applications are received. Interviews are by appointment only.
It is mandatory that volunteers get a flu shot each year.
