FUSE program secures funding through 2026, continuing vital support for chronically homeless patients

Thanks to the generosity of our donors, CHMC Foundation will provide more than $500,000 in funding to continue the Frequent Utilizer System Engagement (FUSE) program at California Hospital Medical Center through fiscal year 2026. This crucial program provides intensive case management, housing navigation and connections to  health care services for chronically unhoused patients in our community.

tents on side walkSince 2013, FUSE has been instrumental in improving the health and housing stability of our most vulnerable patients, reducing avoidable emergency department visits and hospitalizations. With over 12,000 residents within California Hospital’s  Service Planning Area experiencing unsheltered homelessness, the need for FUSE's services is greater than ever.

This continued funding allows CHMC to maintain and expand its partnerships with John Wesley Health Center and Housing Works, ensuring seamless care coordination and access to housing for those who need it most.
